How to Choose an Assisted Living Residence

La Veta Village Assisted Living, a nonprofit 501(c)(3) organization, wants to help you make an informed decision when selecting an assisted living residence for yourself or a loved one. The process requires careful evaluation—engaging all five senses during visits and trusting your initial impressions. Speak with current residents, request references, and avoid rushing the decision.


Consider the Location

  • Is the facility near family or a trusted support person?

  • Are the altitude and weather conditions suitable for maintaining health?

  • Is it conveniently located near stores and essential services?


Review Provider Agreements and Policies

  • Does the admission policy align with your needs?

  • Are the services clearly outlined in the admission agreement?

  • Have you thoroughly reviewed and understood the financial terms?

  • Will unused rent be refunded if a transfer or discharge occurs?

  • Do you understand the reasons for potential transfer or discharge?

  • Can you choose your medical or healthcare providers for additional services?


Tip: Request copies of admission/discharge policies and house rules to review at your leisure. Take your time to ask detailed questions.


Verify Licensing and Certification

  • Is the facility licensed by the Colorado Department of Public Health and Environment (CDPHE) and in good standing?

  • Is the facility Medicaid-certified?


Assess the Space

  • Are the bedrooms and bathrooms private or shared?

  • Is the space clean and adequate for your belongings?

  • Will it accommodate your mobility needs?

  • Are there private areas for family visits?


Evaluate Safety Features

  • Do bathrooms include safety features such as grab bars?

  • Are hallways and doorways wheelchair-accessible?

  • Is there an emergency call system?


Understand the Care Planning Process

  • Are you involved in creating your care plan?

  • Can family, physicians, or other support persons participate?

  • Are care plans regularly updated to reflect changes in your needs?


Explore Personal Services

  • Does the facility assist with dressing, bathing, toileting, and medication management?

  • How often is bathing assistance provided?


Examine Staff Qualifications

  • What training do the staff and administrators receive?

  • What is the staff-to-resident ratio?

  • Are staff available and awake during the night?

  • Is staff turnover frequent?


Assess Meals and Nutrition

  • Are meals nutritious and tailored to special diets or cultural preferences?

  • Can residents participate in meal planning and access the kitchen?

  • Are snacks, extra helpings, and substitutions readily available?


Look into Social Opportunities

  • Is there an activities coordinator and a written schedule of events?

  • Does the facility help arrange transportation?

  • Are the activities engaging, and are there opportunities to socialize with other residents?


Ensure Clear Communication

  • Does the facility notify family and physicians of unusual events?

  • Is the grievance process clear, and do you feel comfortable speaking with staff?

  • Are community phones available, or will you use a personal mobile phone?


Take Tours and Make Observations

  • Observe the kitchen, pantry, and meals being served.

  • Check for cleanliness, pleasant odors, and happy, engaged residents.

  • Assess staff-resident interactions and staffing levels.

  • Visit unannounced after your initial tour to confirm impressions.
